Admission Requirements

• Child’s Birth certificate
• Up-to-date immunization card.
• Medical certificate (within 3 months).
• Passport photo of the child, parent and that of the emergency contact person
• Parent/guardian ID copy.
• Signed Creative Tots day care agreement.
• Personal items (e.g., diapers, clothes, water bottle, crocs, bathing soap, sweater).
• Consent for 24/7 care.
• Orientation attendance confirmation.
• Initial fee deposit.
• Fully signed registration/ enrolment form
